The first step in receiving Real Life Choices begins by contacting your Regional Division of Developmental Disabilities (DDD) office or your case manager.

Working with your Support Coordinator, you will develop a Plan of Care. When it comes to implementing that Plan of Care, Advancing Opportunities can help. The agency can provide Individual Support and Family Support. We can also provide support and training in the following areas: Daily Living Skills, Job Readiness Training and Job Coaching.
